The segment of Kentucky Route 91 between Cave-In-Rock and Marion begins on the shores of the Ohio River at the Cave-In-Rock Ferry. Travelling generally south, the highway bends briefly east to intersect KY-135 and KY-387 before turning southeast towards Marion.
Continuing southeast, the highway makes its way through north Kentucky forests and the hills that make up the Ohio River Valley as it meanders southward from the River. Approaching Marion, the highway turns due east at the intersection with Old Salem Rd. Inside Marion, the segment of KY-91 between Cave-In-Rock and Marion ends at the intersection with U.S. Route 60 in downtown.
